What is the SAP Evaluation?
A Substance Abuse Professional (SAP) evaluation is a federally required assessment for any employee in a DOT safety-sensitive position who has violated a DOT drug or alcohol regulation — including a failed test, a refusal to test, or a confirmed alcohol violation.
Under federal law (49 CFR Part 40), the SAP is a neutral, independent evaluator — not a disciplinary figure. Our role is to assess the employee, recommend the appropriate level of education or treatment, and determine when the employee is ready to safely return to duty.
We are fully certified and serve employees and employers across all DOT-regulated industries Nationwide, with telehealth available where federally permitted.

What Is a SAP Evaluation?
A Substance Abuse Professional (SAP) evaluation is a federally required assessment for any employee in a DOT safety-sensitive position who has violated a DOT drug or alcohol regulation — including a failed test, a refusal to test, or a confirmed alcohol violation.
Under federal law (49 CFR Part 40), the SAP is a neutral, independent evaluator — not a disciplinary figure. Our role is to assess the employee, recommend the appropriate level of education or treatment, and determine when the employee is ready to safely return to duty.

The SAP Process, Start to Finish
Here is exactly what happens from the moment a violation
occurs to the moment you are cleared to return to work.

Employee Is Referred to a SAP
The employer provides a SAP referral list. The employee chooses their own SAP. We accept direct self-referrals, no employer involvement required to get started.

Initial SAP Evaluation
We conduct a face-to-face or telehealth evaluation and recommends the appropriate level of care. The recommendation is sent to the employer.
03
Employee Completes Program
The employee completes the program recommended by us. We evaluate and recommend; we do not run the programs.

Follow-Up Evaluation
After completing the program, the employee returns to us for a follow-up evaluation confirming compliance and readiness to return to safety-sensitive duties.

Return to Duty Report Issued
We issue the Return to Duty Report and a Follow-Up Testing Plan for the employer. Both documents are included at no extra cost. The employee is cleared to return to work.

VIOLATION OCCURS
Employee Receives a DOT Violation
A failed drug or alcohol test, a refusal to test, or another policy violation removes the employee from safety-sensitive duties immediately.

REFERRAL
Employee Is Referred to a SAP
The employer provides a SAP referral list. The employee chooses their own SAP. We accept direct self-referrals, no employer involvement required to get started.

INITIAL EVALUATION · $375
Initial SAP Evaluation
We conduct a face-to-face or telehealth evaluation and recommends the appropriate level of care. The recommendation is sent to the employer.

EDUCATION OR TREATMENT
Employee Completes Recommended Program
The employee completes the program recommended by us. We evaluate and recommend; We do not run the programs.

FOLLOW-UP EVALUATION · $175
Follow-Up Evaluation
After completing the program, the employee returns to us for a follow-up evaluation confirming compliance and readiness to return to safety-sensitive duties.

— You have the right to choose your own SAP. You are not required to use whoever your employer suggests.
— Your evaluation is confidential. We only share what is federally required.
— This process is not a punishment — it is a federal compliance requirement to support your safe return.
— Telehealth evaluations are available where permitted. Services are offered in English and Spanish.
